ACCIDENT WAIVER AND RELEASE OF LIABILITY AGREEMENT

**I, the participant, hereby acknowledge and agree to the following:**

1. **Assumption of Risk:**

I understand and assume all risks associated with participating in any/all activities with **Off Roster Solutions**, including, but not limited to, risks arising from negligence or carelessness, dangerous or defective equipment, or property conditions.

2. **Physical Fitness:**

I certify that I am physically fit and have sufficiently prepared for participation in the activities. I am not aware of any health-related issues that would prevent me from participating.

3. **Liability Release:**

I waive, release, and discharge **Off Roster Solutions**, its officers, employees, agents, representatives, and volunteers from any and all liability, including but not limited to liability arising from negligence, for death, disability, personal injury, property damage, or any other loss or harm that may occur during my participation.

4. **Indemnification:**

I agree to indemnify and hold harmless **Off Roster Solutions** from any and all claims, demands, and causes of action arising from my participation in the activities, whether caused by negligence or otherwise.

5. **Medical Treatment Consent:**

In the event of injury, accident, or illness, I consent to any necessary medical treatment and assume financial responsibility for any costs incurred.

6. **Photo/Video Release:**

I consent to the use of my likeness in any photo, video, or film for any legitimate purpose by the organizers, sponsors, or assigns.

7. **Acknowledgment of Understanding:**

I certify that I have read and fully understand the terms of this agreement. I am aware that by checking this box, I am waiving my right to sue and accepting the risks and responsibilities associated with participation.
